VITA: (one-page maximum) Please provide a short vita that includes at the minimum, your academic history and describes teaching positions held, including dates. This information will be used to determine eligibility.

PERSONAL STATEMENT: (two-page maximum) We seek a diverse group of participants from a wide range of community colleges. An applicant's interest in the teaching and learning of mathematics at two-year colleges, including the implementation of innovative curriculum and pedagogy, and interest in leadership in the profession are important considerations.
Describe what you hope to gain from participation in AMATYC Project ACCCESS. Concrete examples are helpful.  Please tell us about yourself, addressing these interests and your personal approach to teaching and learning.

LETTER OF SUPPORT: A strong level of institutional support is required for acceptance by AMATYC Project ACCCESS. Give your chair or dean a copy of the information in the Administrator's Materials section.

Please consult FAQ page for more details.
